You will find our Child Care Policies Manual here. The other links are to forms that need to be completed before your child's first day of school.
The following paperwork is only required of families who have already completed a tour and been accepted into our program.

We have earned the top rating from Parent Aware (4 Star Rating). We encourage you to visit their website. There are many free tools and resources available on their website, including help on paying for child care.
child care policies manual
Our Child Care Policies Manual is updated every year and is always available here for your convenience. Any questions regarding our policies should be discussed with our Director, Katie Rosenthal.
admissions and arrangements
This form from the Minnesota Department of Human Services is required to be kept on file and up-to-date for each child in our care.
immunization form
You may fill out this form or attach a copy of your child's immunization record.
allergy form
This form only needs to be filled out if your child has allergies. If your child is allergy-free, please skip this form.
medication form
Specify which over-the-counter and prescription medication you authorize Hidden Valley Preschool to administer.
WADING POOL PERMISSION FORM
This form is only required for the summer session. You can authorize your child to use our wading pools.
photo release form
Allow or decline the use of your child's photos on various platforms.
getting to know you
Introduce us to your child so we can quickly meet their needs on day one.
recurring payment authorization
Optional. You may choose to keep a bank account on file for monthly tuition payments and session deposits. If you do not wish to use this option, you may submit tuition via check or cash.
